Dr. Adams is a fellow of the American College of Mohs Surgery, a qualification earned by fewer than eight percent of all dermatologists nationwide. He has been performing Mohs Surgery for over 15 years on the Gulf Coast and is one of only three fellowship-trained Mohs surgeons serving the greater Emerald Coast region.

How to Care for Your Wound – Post Biopsy Instructions

Our Board Certified Physicians use the most advanced treatment techniques available in skin care and surgery to deliver the best results possible. But what happens once you return back to your every day routine? Find out how to care for your biopsy site once you have received treatment and are released.

Post Biopsy Instructions

Change the Dressing Once Daily:

After 24 hours, wash your hands and remove original bandage.
Gently clean the biopsy site(s) with a gentle soap and warm water.
Rinse with clean warm water and pat the area(s) dry with gauze or a dry cloth.
Apply a thin layer of Vaseline ointment with a clean Q-Tip.
Apply a clean band-aid to the area.

** If you have sutures, continue the care every day until you have them removed.

What to Expect:

Swelling and redness may occur around the wound for several days after the biopsy.
A low-grade fever (99.0 – 101.0 °F) may occur and can be treated with acetaminophen (Tylenol).
Light drainage is to be expected. The draining may be white, yellow, or light green.
Avoid hot tubs, swimming pools, and soaking in water for long periods of time.
If bleeding occurs, apply firm constant pressure to the dressing for 30 minutes — NO peeking! If bleeding persists, please call the doctor. If you are unable to reach the doctor, go to the Emergency Room.

A Normal Healing Biopsy Site:

There may be redness at the site, but unless it is expanding or the site is increasingly sensitive, it is most likely normal healing. White or yellow in the center is also normal unless there is increasing drainage or a foul odor.

We will attempt to contact you within 2 weeks with biopsy results. If you have not heard from us after 2 weeks, please call the office at (850) 654-3376.

Excisional skin surgery is defined as the surgical removal of either a benign or malignant skin lesion. Excisions are performed in the office under the safety of local anesthesia. The surgery is usually performed with a scalpel and removes the lesion with a margin of normal skin and underlying tissues. The size of the margin often depends on whether the lesion is benign or malignant. After the lesion has been excised, the surgical site may be closed with sutures or it may be ideal to allow it to heal on its own (referred to as healing by secondary intention).

Before any skin surgical procedure is performed, the dermatologist or physician assistant will review your medical background for safety and will discuss the different treatment and closure options with you. Upon the completion of the surgery, you will receive both written and verbal instructions and will most likely be scheduled to return for a follow-up visit in one to two weeks.

Dr. Adams, a fellowship-trained Mohs surgeon who has the extensive expertise and training to effectively treat many types of skin cancer, took a moment to answer some frequently asked questions about Mohs Microscopic Surgery.

He has participated in over 700 Mohs surgical cases in training and he has performed more than 14,000 Mohs procedures in private practice. Dr. Adams is a Fellow of the American College of Mohs Surgery, a distinction earned by fewer than 8% of all dermatologists in the United States.

Q. What is Mohs Micrographic Surgery?

A. While some skin cancers can be adequately treated through standard excision by a dermatologist, some skin cancers require a more precise surgical technique. Mohs micrographic surgery is an outpatient based surgical method that removes skin cancer while simultaneously preserving all of the normal surrounding skin. In properly trained hands, Mohs surgery has the highest cure rate of any other treatment of skin cancer. Mohs surgery is typically performed in areas of the body where preserving normal skin tissue is critical, such as the face, eyelids, nose, ears, and lips. By preserving all of the normal skin, the Mohs surgical defect remains as small as possible. Smaller defects require fewer stitches and less complicated pairs and ultimately result in quicker healing and optimal cosmetic and functional outcomes. Mohs micrographic surgery is performed in the office under the safety of local anesthesia.

Q. What skin condition does this procedure treat?

A. Mohs micrographic surgery treats a variety of non-melanoma skin cancers, most commonly basal cell carcinoma and squamous cell carcinoma. Mohs micrographic surgery is also used to treat less common cancers such as atypical fibroxanthoma, sebaceous carcinoma, dermatofibroma sarcoma protuberans, micro-cystic adnexal carcinoma, and a few others.

Q. How long does the surgery last?

A. The duration of most micrographic surgery varies; however, patients generally can expect to spend from one to three hours at our office for their procedure. Despite the duration, patients are usually able to eat and drink and relax as normal while at the office having their procedure.

Q. What is the recovery time after surgery?

A. After having Mohs micrographic surgery, patients should allow adequate time to recover and recuperate. In nearly all situations, patients should expect to take it easy for the first 48 hours after their surgery. Every patient will leave the office with pressure dressing in place. They will be asked to keep this pressure dressing clean and dry for the first two days. Some patients may be advised to sleep with an extra pillow under your head at night. After approximately 48 hours, every patient will be allowed to “take the million-dollar shower”, get the dressing wet, and change the dressing. For surgical sites that are sutured, the recovery time is generally 5 to 7 days.

Some surgical sites achieve optimal cosmetic and functional results by healing without stitches. This process is termed “healing by secondary intention”. When surgical wounds heal by secondary intention, they may heal within one week or in certain circumstances they may require a few months.

Q. Can Mohs surgery be performed to treat all skin cancers?

A. Mohs surgery is not indicated for the treatment of all skin cancers. Mohs micrographic surgery is indicated for the treatment of skin cancers involving the face, eyelids, ears, nose, and lips. Mohs micrographic surgery can also be used to treat skin cancers involving the genitalia and other high-risk sites such as the lower legs and fingers and hands. Mohs micrographic surgery is also appropriate to treat aggressive skin cancers and skin cancers that have recurred after prior unsuccessful surgical treatment.

Q. Why should a dermatologist that performs Mohs surgery be fellowship trained?

A. Only 8% of dermatologists in the United States have completed a fellowship in Mohs micrographic surgery. Dermatologists who successfully complete a fellowship in Mohs micrographic surgery have received extensive training in the various methods of this procedure ranging from the basic to the most complex tumors and situations as well as advanced training in surgical reconstruction methods. The advanced certification of Fellowship trained Mohs surgeons is recognized by the presence of “FACMS” (Fellow of the American College of Mohs Surgeons) in their professional title.

The Benefits of Mohs Micrographic Surgery

The founder and originator of Mohs micrographic surgery, Frederic E. Mohs, M.D. (1910-2002), established a specialized technique of surgical skin cancer removal that has revolutionized the modern day treatment of skin cancer. Mohs Micrographic Surgery allows for optimal cancer cure rates, maximal preservation of all of the normal skin, faster healing, and optimal cosmetic and functional outcomes.

As stated in a recent article published in The Skin Cancer Foundation Journal, Mohs surgery is on the rise, for good reasons: It has the lowest recurrence rates, highest cure rates and best cosmetic results of any skin cancer treatment.

Only Fellowship-Trained Mohs Surgery is offered at Coastal Skin Surgery & Dermatology along the Emerald Coast. David Adams, M.D., FACMS is one of the region’s leading Fellowship-Trained Mohs Surgeons and has performed over 15,000 Mohs procedures in private practice.

Dr. David Adams specializes in the Mohs procedure and advanced facial reconstructive surgery. He has experience with numerous types of nonmelanoma skin cancer, including rare tumor types and complex locations.

Non-melanoma skin cancer generally refers to Basal Cell Carcinoma and Squamous Cell Carcinoma. Non-melanoma skin cancer is the most common type of cancer, and is felt to be largely caused by over-exposure to the sun’s ultra-violet rays. Non-melanoma skin cancer can be easily treated, especially when identified and diagnosed early.

Although a single procedure may take several hours, the Mohs process examines 100 percent of the tissue margins under the microscope. Only 1 percent of the tissue margins are examined microscopically in traditional surgical excision. With the preservation of all of the normal skin and a cure rate over 98 percent, Mohs Micrographic Surgery continues to be a front-line treatment for nonmelanoma skin cancers.
